Checklist for Volunteers and Coaches

Thank you SO MUCH for volunteering your time and expertise to the young athletes of Glendora Youth Soccer. We are thrilled to have you join our team. 

There are a few things that MUST be completed prior to you taking the field with your team:

  1. If you have not done so, Log In to the website
  2. Print and fill out Glendora's Volunteer Application.  (this must be done every year) Volunteer APP
  3. If you did not do this last year, view the online video, download and print your certificate of completion for the CDC's Heads Up Concussion Training,   If you did this last year, pull up your cert from last year and upload it for step 7
  4. If you did not do this last year,  view and/or participate in a CPR training program (online, in-person, or both). CPR Online Course Download and print the CPR BASICS, QUIZ, and Declaration of CPR Training attendance form, OR provide a copy of your valid CPR card, If you did this last year, pull up your cert from last year and upload it for step 7
  5. Complete the Mandated Reporter training https://mandatedreporterca.com/training/volunteers
  6. Links to all of this info are accessed by clicking on the green text above.  Once you have these documents, complete the training or fill out the forms, scan or take pictures of them and save them on your computer.

  7.  Register as a volunteer  and upload the scans or pictures of the above documents as necessary.  NOTE: the document uploads are required to register, so complete and have the info ready to upload before clicking the following link  https://glendorayouthsoccer.sportngin.com/register/form/888993262

  8. Fingerprinting is now required.  Once you have finished your registration, you will be contacted via email and provided with further information concerning the fingerprinting process. Please print and fill out this form to take with you: Request for Live Scan Service
  9. All of the resources for training and forms can be found on this page (see below).

After you have completed your CPR, Concussion Training, and Volunteer Application form, please be sure to access your profile and edit your volunteer registration to upload the confirmations.
